The MSIDS Questionnaire

The MSIDS Questionnaire is designed to identify patterns in chronic Lyme disease and associated co-infections often missed in traditional evaluations.  helping uncover the multiple factors that may be contributing to chronic symptoms.

By evaluating a wide range of symptoms across systems, this questionnaire helps identify patterns that may point to underlying drivers of chronic illness including infections, immune dysfunction, inflammation, and environmental factors.

The questionnaire takes approximately 10–15 minutes to complete.
